 Self-Assessment Questionnaire: A Brief Introduction

In an effort to help states evaluate their Medicaid and CHIP enrollment and renewal processes, this module includes an interactive web-based questionnaire. The questionnaire is designed to help determine strengths and opportunities for improvement in states’ enrollment and renewal processes across six dimensions:  

  • enrollment and renewal simplifications;
  • inter-agency coordination;
  • analytic capacity for program management and decision making;
  • organizational culture;
  • community partnerships and outreach; and
  • state leadership.

After completing the questionnaire this interactive tool will provide a customized report that indicates areas of strength and opportunities for improvement in their enrollment and renewal processes, based on available literature and experiences that other states have had with enrollment and retention strategies.   This questionnaire is detailed and may require the expertise and knowledge of different people within a state’s Medicaid or CHIP agencies. So, states have the ability to complete the questionnaire in multiple sessions, saving work throughout the process by establishing a log-in ID and password. Each state will have one log-in that can be shared with agency colleagues as necessary. This module primarily is for use by state Medicaid and CHIP agency leaders, but they can seek input from stakeholder partners on some of the questions.
